Nurse practitioners authorization

The director authorizes Oregon licensed nurse practitioners if they certify to the director that they have reviewed and understand informational materials provided by the Workers' Compensation Division (WCD) about the workers' compensation system.

You must maintain an Oregon nurse practitioner license to receive the nurse practioner authorization. If you treat a patient with an Oregon workers' compensation claim, and you are not authorized by the director, the insurer will not have to pay for the services you provided.

An introduction to the American College of Occupational and Environmental Medicine's disability prevention guidelines titled, "Preventing Needless Work Disability by Helping People Stay Employed"

The Employer-at-Injury Program (return to work) information

After you have read the above materials, please complete the "Statement of Authorization" online form.
