Filing Complaints

Consumers

If you have problems with an insurance company or agent, please complete a complaint form so we can look into your situation.

As advocates, we want to help all consumers and medical providers. However, the Insurance Division does not have jurisdiction in all cases. We will let you know if your case belongs with a different agency or a court.

Medical providers

If you are a medical provider and are having an issue with one insurer and multiple patients, please contact our office by phone for specific directions. 1-888-877-4894.
For in-network providers, we ask you to contact your provider relations representative to resolve the issue. If you are unable to reach a resolution, please complete the online complaint form below.

If you are an out-of-network medical provider, please complete a complaint form so we can look into your case.

What happens when we receive a complaint?

An advocate will:

  • Provide written verification that your complaint was received
  • Provide a copy of your complaint to the insurance company and/or agent
  • Obtain a detailed response from the company and/or agent, including any supporting documentation to support the position taken
  • Analyze the response and supporting documentation
  • Determine whether there has been a possible violation
  • Advise you of our findings

Please be advised, we are not attorneys and we cannot act as a legal representative. As advocates, we make every effort to determine whether the company/agent complies with the insurance laws or terms of the insurance policy.

Our intent is to find a resolution to your concerns. Results vary from providing clarifying information to getting a claim paid.

How long will it take?

Most complaints are resolved within 60 days but it depends on the type of complaint.

Confidentiality

Complaints are confidential. However, a copy will be sent to the insurance company/agent named in the complaint.
