Filing a fingerprint card and criminal records request form with the Insurance DivisionFingerprint CardsOregon law requires prospective insurance producers to file an official fingerprint card when they submit their license application to the Insurance Division. Because the card must meet certain specifications not controlled by the division, it is not practical to make one available on-line at this time. To obtain a card, insurance producer applicants should contact the Insurance Division's Producer Licensing Section by telephone at 503-947-7981, by e-mail at web.insagent@state.or.us, or by conventional mail at: Insurance Division - 3 Criminal Records RequestThe criminal records request form must be submitted, as well as the fingerprint card, in order for the application to be processed.
