The Health Insurance Reform Advisory Committee (HIRAC) unanimously agreed that insurers
should modify both the prevailing and low-cost benefit designs of the insurance and managed
care portability policies to reflect the requirements of the PPACA.
A new product standard form 440-4902
was created and posted to our filing
requirements website to use when submitting a Portability filing.
