Product Standards Review Cycle

The Division has implemented a product standard review schedule. If new laws or legislation occur before the scheduled review date the product standard will be revised as soon as possible. The review schedule is located on the current forms list.

We will accept comments, requests for corrections, or clarifications at anytime. Please email any comments or requests to dcbs.ratesforms@state.or.us.

In your message please include the following:

• Product Standard Form number with edition date
• Form name
• Page number
• Category and Reference
• Explanation of what needs to be corrected or clarified and why.
