Licensing Procedures – Consultants
Consultants may be licensed for life, health, or general lines (property & casualty). Consultants
may charge a fee for providing insurance advice.
Individuals
Oregon Resident
To apply as a resident consultant:
- Submit an Individual Insurance License Application (Form
3000).
- Submit a fingerprint card (Form FD-258).
- Submit the Criminal Records Request form (Form
4862).
- Provide proof of $500,000 errors and omissions insurance covering consultant
activities.
- Pay a $119.50 fee.
NOTES:
- The applicant must have five years of current insurance experience.
- The exam is waived for currently licensed producers.
NOTE: Beginning January 1, 2011, all fingerprinting will be performed at the testing centers. Fingerprint candidates should call the exam vendor to set up an appointment to have their fingerprints digitally scanned and transmitted to the Oregon State Police for processing.

Exams are administered by Prometric. See PSI's
website (PSIexams.com) for information for candidates for Oregon licenses, including how to register
for exams, exam outlines, and exam fees. An insurance application must be approved before an
applicant can register for an exam. Exam fees are paid directly to PSI at the time of exam registration.
Individuals who wish to request special accommodations under the Americans with Disabilities
Act (ADA) or for whom English is a second language (ESL) may request special exam arrangements
by submitting a special accommodations form to PSI. The form is available on PSI's website in
the licensing information bulletin.
Individuals
Nonresident
To apply as a nonresident consultant:
- Submit an Individual Insurance License Application (Form
3000).
- Submit an original letter of certification, not older than 90 days, indicating the applicant
is licensed in his or her home state as a consultant.
- Provide proof of $500,000 errors and omissions insurance covering consultant activities.
- Pay a $75 fee.
NOTE: The applicant must have five years of current insurance experience.
Business Entities
Oregon Residents
To apply as a resident consultant business entity:
- Submit a Business Entity Application (Form
3001).
- Pay a $75 fee.
Business Entities
Nonresidents
To apply as a nonresident consultant business entity:
- Submit a Business Entity Application (Form
3001).
- Submit an original letter of certification, not older than 90 days, indicating the applicant
is licensed in his or her home state as consultant.
- Pay a $75 fee
NOTE: The business entity (agency) also must file an affiliation of producer
(Form
2139) within 30 days.
