Licensing Application Procedures

Business Entity Producers

The following are required to be licensed as a business entity:

  • corporation
  • association
  • partnership
  • limited liability company
  • limited liability partnership or other legal entity

NOTE: Sole proprietors are not required to be licensed. However, some insurance companies may require a sole proprietor to become licensed as a condition of the agency contract.

Oregon Residents

To apply as a resident business entity:

  1. Submit a Uniform Application for Business Entity Insurance License/Registration.
  2. Pay a $75 fee.
  3. The business entity (agency) also must file an affiliation of producer (Form 2139) within 30 days.

NOTE:

  1. Assumed business names must be registered with the Secretary of State Corporation Division.

Nonresidents

To apply as a nonresident business entity:

  1. Submit a Uniform Application for Business Entity Insurance License/Registration.
  2. Pay a $75 fee.
  3. The business entity (agency) also must file an affiliation of producer (Form 2139) within 30 days.
