  1. You must be an Oregon employer.
  2. You must sign up on or before May 1, 2012, to be eligible. It's important to include a contact person, e-mail address, employer name, and description of Safety Break activities.
  3. You must hold a Safety Break event (at discretion of employer) on May 9, 2012, to be eligible.
