INSTRUCTIONS FOR FILING REQUESTS FOR HEARING/BOARD REVIEW BY E-MAIL

1. Send an e-mail to: request.wcb@state.or.us
2. Attach an electronic copy of a completed WCB Hearing Request or WCB Request for Board Review form (found on this website under "forms" or click here).
3. Attach an electronic copy of a certificate of service by mail, fax or e-mail.
4. A "cc" of the e-mail and the attachment should also be sent to the e-mail addresses of the other parties and their attorneys.
5. Parties may also file Request for Hearing and Request for Board Review in-person, by mail, or by FAX 503-373-1600.
